Client must be confined to home or have taxing effort to leave his/her place of residence.

The client must be under the care of a licensed physician or willing to receive care from a physician.

The service must be skilled and must be deemed reasonable and necessary according to Medicare, Medicaid or insurance rules and regulation.

There must be a reasonable expectation that the medical needs of the client can be met adequately in his/her residence.
